- I understand that in registering for the program to receive services and financial assistance, my personal details will be recorded on a database that may be accessed by Joblink Midwest as the primary funding body and DTWD as a Jobs and Skills Centre client. This information may be used for the verification of services provided and funding allocated.
- The personal details I provide may also be used as part of a review process to gain confidential feedback on the benefits, and value, I have gained from Joblink Midwest. By signing this declaration, I agree to participate in such a review process, and any other review process determined by Joblink Midwest as necessary..
- I understand that I am not entitled to a specific amount of funding for training. The amount of funding I receive will be linked to my personal Career Action Plan
- I understand that Career Boost Funding will under no circumstance be allocated retrospectively. All training or services for which Career Boost Funding is to be sought will need to be approved by a Joblink Midwest career practitioner prior to enrolment or commitment of funds. Payments will only be made directly to the service provider. Retrospective reimbursements will not be possible.
- I understand that I cannot be assisted with funding for any training activity where Career Boost Funding can be successfully secured through an alternative source icluding Workforce Australia or an NDIS service.
- I agree to advise Joblink Midwest of any funding that I receive, or can access, to support the career development activities outlined in my personal Career Action Plan.
